COVID-19 VACCINATION

All eligible patients from the 8 PCN Practices are currently being invited to attend the Hull KCOM Stadium for a Covid-19 vaccination at one of our Symphonie PCN arranged clinics whilst eligible housebound patients are being vaccinated by the Home Visiting Team.

Patients that have provided a mobile telephone number when registering with their GP Surgery receive a text message with a link to book an appointment slot whilst those without a mobile number receive a telephone call to book. Our team may be working from home therefore some calls to patients may come through withholding the telephone number.  We are aware there are multiple scams circulating claiming to be offering a vaccination however our invitation link will only require patients to confirm their date of birth.

CARE COORDINATORS

Zoe Nicholson and Sarah Taylor joined the Symphonie PCN team in November 2020.

 

As Care Coordinators they are responsible for helping monitor and coordinate patient care plans, providing support and connecting patients with relevant health care providers and the home visiting team.

Due to the current pandemic their roles have been temporarily adjusted to focus on rolling out the Covid-19 vaccinations to all eligible patients, coordinating vaccination clinics and arranging home visits for housebound patients and providing support to any patients who unfortunately test positive for coronavirus and distributing pulse oximeters to enable patients to detect any deterioration in their oxygen levels and provide advice on when to seek urgent help.

 We held a ‘drop in’ for staff, patients, local trusts and community services on 13/9/19 to meet the Team and gain an understanding of our aims. The event was successful and well attended.

Social Prescriber- Victoria Whittles

Commencing 4/11/19

 

We are pleased to announce we have Victoria Whittles joining our teams to provide valuable support to patients with a wide range of social, emotional or practical needs, and improve well-being.

Vicky has worked in the community and voluntary sector for the last fifteen years and has a wealth of experience working with the public. Most recently Vicky has worked for Healthwatch Hull and Connect Well as a well-being co-ordinator, giving her the best possible foundation for working as a link worker. 

Vicky has a passion for helping others and is looking forward to working with Symphonie patients across the city. When Vicky is not social prescribing, she is an avid Hull FC fan and can frequently be found in a cold, wet KC stadium on a Friday evening.
